We are seeking an experienced and skilled Clinical Lead Nurse to join our team at Leaders in Care as a Paediatric Nurse Manager. This is an exciting opportunity to work with a well-established provider of complex care packages within the community.
About the Role:The successful candidate will be responsible for providing face-to-face support to children and their families, creating personalised care plans, and working closely with a core team of carers to train and support them with their clinical skills and activities.
Key Responsibilities:- Design and Deliver Care Plans: Develop and implement tailored care plans specific to individual client needs, ensuring effective management of their physical and physiological requirements.
- Lead Multidisciplinary Teams: Effectively manage and coordinate the work of multidisciplinary teams through strong communication and teamwork, ensuring seamless delivery of care services.
- Client Support and Assessment: Provide ongoing monitoring, support, and assessment of client needs, identifying areas for improvement and implementing changes as necessary.
- Nursing Interventions: Deliver nursing interventions, including the management of medicine delivery, to ensure optimal client outcomes.
- Relationship Building: Build and maintain sophisticated relationships with clients and their families, ensuring their needs are met and their expectations are exceeded.
- Collaboration and Partnership: Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care, ensuring effective coordination and delivery of services.
- RSCN with a minimum of 6 months' experience in the UK and an active NMC PIN.
- Supportive management style that can bring out the best in others.
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years.
- Advanced communication skills verbally and written to liaise effectively with patients, families, and multidisciplinary teams.
- Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs.
- Full UK Driving Licence desirable but not essential.
- Up to £50,000 per annum DOE.
- 2 days working from home per week / 3 days in the community.
- Monday to Friday (9-5) hours.
- 33 days holiday.

- Business travel expenses of £0.45 per mile.
