Position Overview: We are excited to present an opportunity for an individual to become a vital member of a dedicated team, consisting of 4 staff members and 12 individuals with diverse care and support requirements. The role of Lead Care Facilitator involves providing essential support to individuals diagnosed with persistent mental health conditions.

Service Environment: The service operates within a supportive accommodation setting, comprising 12 individual flats along with communal areas and a designated staff office. This environment is designed to foster a sense of community while ensuring that all residents receive the necessary support.

Key Responsibilities:

  • Oversee daily operations, including managing staff schedules and addressing staffing needs as they arise.
  • Collaborate closely with the Service Manager and Support Workers to ensure that personalized support is delivered in accordance with individual care plans.
  • Provide direct supervision and guidance to support staff, ensuring adherence to best practices in care delivery.
  • Engage in regular supervision sessions with your line manager and participate in ongoing training to enhance your professional skills and knowledge.
  • Take responsibility for audits in areas such as Health and Safety, incident reporting, safeguarding, and service user outcomes.

Approach to Care: You will work directly with individuals using trauma-informed and recovery-oriented methodologies, empowering them to lead fulfilling lives, engage with their communities, and develop their skills. Your role will also involve assisting individuals in building tenancy-related skills and managing their housing effectively.
