Job Summary

We are seeking a highly skilled and motivated individual to join our Integrated Discharge Team as a Senior Ward Discharge Coordinator. This role will involve working collaboratively with internal and external partners to actively monitor and case manage the discharge plans for patients with complex care and support needs.

Main Responsibilities
  • Develop and implement effective discharge plans to ensure timely and safe discharge of patients.
  • Collaborate with the multi-disciplinary team to gather and collate information to support discharge planning.
  • Provide updates and coordinate discharges to improve joint working practices and patient care.
  • Ensure accurate and up-to-date documentation of patient discharge plans and progress.
  • Work closely with the ward team and IDT to ensure seamless discharge processes.
Requirements
  • GCSE level (or equivalent) in English and Maths.
  • Experience of communication with the general public in a working environment.
  • Good communication skills and ability to interact with patients, relatives, and team members.
  • Attention to detail and accurate presentation.
  • Ability to work flexibly to meet the needs of the service.
