About the Role

We are seeking a highly skilled and motivated Registered Nurse to join our Community Access Team at Norfolk Community Health and Care NHS Trust. As a key member of our team, you will play a vital role in providing high-quality patient care and supporting the Admission Avoidance pathways in the Emergency Department and the Older Persons Emergency Department.

Key Responsibilities
  • Provide patient-focused care, ensuring smooth planning, coordination, and communication between services to facilitate seamless care and identification of gaps in the present systems.
  • Link directly with wards, acting as a resource to the multi-disciplinary team, patients, and their carers, to support safe and timely discharge.
  • Work across organisational boundaries, ensuring open, honest, and effective communications are maintained to facilitate joint working practices around integrated patient pathways for transfer of care.
  • Proactively and positively work with colleagues within the Integrated Discharge Team to reduce delays in transfers from the Acute sector.
  • Work with Emergency and Intermediate Care areas to provide holistic patient assessment to avoid admission for patients who do not require acute intervention.
Person Specification
  • Registered General Nurse with current NMC registration.
  • Evidence of continuing professional development.
  • Assessor/mentorship preparation; ENB 998 or equivalent (desirable).
  • Evidence of working at relevant level within the community setting.
  • Evidence of working at a relevant level.
  • Knowledge of additional specialist areas through in-house training & short courses (desirable).
Requirements
  • The post holder will be required to work shifts over a seven-day period.
  • The post holder will need to be passed fit to perform full duties of the post through occupational health with any reasonable aids and adaptations if necessary.
  • Able to communicate effectively in written and verbal English Language.
  • Kneel, bend & stoop, and work in cramped environments.
  • Manoeuvre limbs of around 5-6 kg.
  • Manoeuvre patients using handling aids.
