Position Overview

We are seeking a motivated and proactive individual to become a vital member of our Integrated Discharge Team as an Administrative Support Coordinator. This role involves working closely with both internal and external stakeholders, diligently tracking and entering data, and facilitating the discharge processes for patients with intricate care and support requirements.

The successful candidate will be part of a well-established team, dedicated to enhancing patient flow and care coordination. Please note that this position requires on-site presence, as remote work options are not available.

Strong organizational abilities are crucial, along with the capacity to manage competing priorities effectively. Proficiency in IT is essential, as the role involves handling various data sources, including spreadsheets, emails, and specialized software such as SystmOne and OPTICA (Optimised Patient Tracking and Intelligent Choices Application). This system allows for comprehensive tracking of all patients referred to the Integrated Discharge Team.

Timely and appropriate patient discharge is essential for maintaining operational efficiency within the healthcare facility, and comprehensive training will be provided to ensure success in this role.

Key Responsibilities

As the Administrative Support Coordinator, your responsibilities will include:

  • Delivering essential administrative assistance to the multidisciplinary team to enhance patient care within the Integrated Discharge Team.
  • Managing data, information, spreadsheets, and reports to support the ongoing efforts of the Integrated Discharge Team, contributing to the overall enhancement of discharge services across the organization. This includes data extraction from various systems.
  • Collaborating effectively within a multidisciplinary team environment.
  • Receiving training on Trust Systems, including PAS, Medworxx, and ICE, as well as Microsoft Word, Excel, and Teams.
  • Being supported in your career advancement within the healthcare sector.

Job Responsibilities

  • Collecting and interpreting comprehensive and accurate data for the Integrated Discharge Team.
  • Managing data and information to assist the Integrated Discharge Team in sustaining its operations, contributing to the overall improvement of discharge services across the organization.
  • Monitoring referrals and triaging information within the Integrated Discharge Team in a timely manner.
  • Participating in daily triage meetings, updating relevant reports during the meeting, and distributing them upon completion.
  • Ensuring that patient tracking spreadsheets are consistently updated.

Qualifications

Essential

  • A good standard of numeracy and literacy, equivalent to GCSE level education.

Experience

Essential

  • Some administrative experience.
  • Proficient in Microsoft Office and familiar with PAS, Medworxx, ICE, and SystmOne.

Desirable

  • Experience within the NHS or healthcare sector.

Skills

Essential

  • Ability to create and utilize databases and spreadsheets, along with excellent general IT skills.
  • Capacity to prioritize workload and meet deadlines effectively.
