Job Summary

We are seeking a highly skilled and motivated individual to join our Integrated Discharge Team as a Senior Ward Discharge Coordinator. This role will involve working collaboratively with internal and external partners to actively monitor and case manage the discharge plans for patients with complex care and support needs.

Main Responsibilities
  • Develop and implement effective discharge plans to ensure timely and safe discharges for patients.
  • Collaborate with the multi-disciplinary team to gather and collate information to enable a clear plan for discharge to be followed from point of admission.
  • Provide updates, coordinate discharges, and provide administrative support to the multi-disciplinary team within the hospital and system partners.
  • Ensure ward view, OPTICA, and handover remain up to date with any discharge progress.
  • Assist in ensuring that all patients have an accurate EDD identifying if they meet Criteria to Reside and reason for delayed discharge reason in line with National Discharge policy.
Requirements
  • GCSE level (or equivalent) in English and Maths.
  • Experience of communication with the general public in a working environment both in person and on the telephone.
  • Good communication skills and the ability to interact appropriately with patients, relatives, team members, and other staff groups.
  • Attention to detail and accurate presentation.
  • Ability to work flexibly in order to meet the needs of the service.
  • Enthusiastic and self-motivated.
