Job Summary

We are seeking a highly motivated and organized individual to join our Integrated Discharge Team as a Senior Ward Discharge Coordinator. This role will involve working collaboratively with internal and external partners to monitor and case manage discharge plans for patients with complex care and support needs.

Main Responsibilities
  • Develop and implement discharge plans for patients with complex needs, working closely with the multidisciplinary team and external partners.
  • Provide administrative support to the discharge team, including updating patient records and coordinating discharges.
  • Work closely with the ward team to ensure timely and safe discharges, and to identify and address any barriers to discharge.
  • Collaborate with external partners, including social services and community healthcare teams, to ensure seamless transitions of care.
  • Provide education and support to patients and their families on discharge planning and care coordination.
