Job Summary

We are seeking a highly skilled and motivated Senior Ward Discharge Coordinator to join our Integrated Discharge Team at the Norfolk and Norwich University Hospital. As a key member of our team, you will play a vital role in ensuring the safe and timely discharge of patients with complex care and support needs.

Main Responsibilities
  • Develop and implement effective discharge plans for patients with complex needs, working closely with the multidisciplinary team and external partners.
  • Collaborate with healthcare professionals to identify and address barriers to discharge, ensuring patients receive the necessary support and care.
  • Provide administrative support to the discharge team, including updating patient records and coordinating discharge arrangements.
  • Work closely with the ward team to ensure seamless discharge planning and execution, minimizing delays and improving patient flow.
  • Develop and maintain strong relationships with external partners, including social care agencies and community services, to ensure effective discharge planning and coordination.
Requirements
  • GCSE level (or equivalent) in English and Maths.
  • Experience of communication with the general public in a working environment, both in person and on the telephone.
  • Good communication skills and the ability to interact appropriately with patients, relatives, team members, and other staff groups.
  • Attention to detail and accurate presentation.
  • Ability to work flexibly to meet the needs of the service.
