Job Summary

We are seeking a highly skilled and motivated Discharge Coordinator to join our Integrated Discharge Team at the Norfolk and Norwich University Hospital. As a key member of our team, you will play a vital role in ensuring the safe and timely discharge of patients from our hospital.

Main Responsibilities
  • Develop and implement discharge plans for patients with complex care needs, working closely with the multidisciplinary team and external partners.
  • Collaborate with healthcare professionals to identify and address barriers to discharge, ensuring patients receive the necessary support and resources.
  • Provide administrative support to the discharge team, including updating patient records and coordinating discharge arrangements.
  • Work closely with patients, families, and carers to ensure they are informed and involved in the discharge process.
  • Contribute to the development and implementation of discharge policies and procedures, ensuring they are aligned with hospital and national guidelines.
Requirements
  • GCSE level (or equivalent) in English and Maths.
  • Experience of working in a healthcare setting, with a focus on discharge planning and coordination.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours.
  • Knowledge of hospital discharge processes and procedures, including D2A and Home First.
