About the Role

We are seeking a highly skilled and motivated individual to join our Integrated Discharge Team as a Senior Ward Discharge Coordinator. This is an exciting opportunity to work collaboratively with internal and external partners to actively monitor and case manage the discharge plans for patients with complex care and support needs.

The successful candidate will have excellent communication skills, the ability to work autonomously, and a good working knowledge of hospital discharge processes. They will be responsible for gathering and collating information from medical notes, patients, and multi-disciplinary team colleagues to enable a clear plan for discharge to be followed from point of admission.

Main Responsibilities

  • To have an up-to-date knowledge of the multidisciplinary management of patients and discharge plans, alongside complex case support.
  • To have a good understanding of Discharge to Assess, Home First, and Discharge Pathways 1-3, as identified in the hospital discharge and community support guidance.
  • To have an understanding of clinical conditions and terminology.
  • To independently gather and collate information from medical notes, patients, and multi-disciplinary team colleagues to enable a clear plan for discharge to be followed from point of admission.
  • To provide updates, coordinate discharges, and provide administrative support to the multi-disciplinary team within the hospital and system partners.

Person Specification

Essential

  • Experience of communication with the general public in a working environment, both in person and on the telephone.
  • Good communication skills and the ability to interact appropriately with patients, relatives, team members, and other staff groups.
  • Attention to detail and accurate presentation.
  • Ability to work flexibly in order to meet the needs of the service.
