About the Role

We are seeking a highly skilled and motivated Senior Ward Discharge Coordinator to join our Integrated Discharge Team at the Norfolk and Norwich University Hospital (NNUH). As a key member of the team, you will play a vital role in ensuring the safe and timely discharge of patients from our wards.

Main Responsibilities

  • To have a good understanding of Discharge to Assess, Home First, and Discharge Pathways 1-3, as identified in the Hospital Discharge and Community Support Guidance.
  • To have an up-to-date knowledge of the multidisciplinary management of patients and discharge plans for all patients referred on D2A P1-3, alongside complex case support.
  • To independently gather and collate information from medical notes, patients, and multi-disciplinary team colleagues to enable a clear plan for discharge to be followed from point of admission.
  • To provide updates, coordinate discharges, and provide administrative support to the multi-disciplinary team within the hospital and system partners to improve joint working practices leading to more effective patient care and timely discharges.

  • Good communication skills and the ability to interact appropriately with patients, relatives, team members, and other staff groups
  • Attention to detail and accurate presentation
  • Ability to work flexibly in order to meet the needs of the service
