At Norfolk and Norwich University Hospital, we are seeking a highly skilled Discharge Coordinator to join our Integrated Discharge Team. This exciting opportunity will enable the successful candidate to work collaboratively with internal and external partners to monitor and case manage discharge plans for patients with complex care and support needs.

The ideal candidate will possess excellent communication skills, be able to work autonomously, and have a good working knowledge of hospital discharge processes (D2A). They will also demonstrate the ability to remain calm under pressure and effectively prioritize tasks.

This role offers the chance to work as part of a strong multidisciplinary team, contributing to the safe and timely discharge of patients. The Discharge Coordinator will play a key role in supporting early conversations and referrals to ensure patients are ready for discharge when no longer meeting criteria to reside.

The salary for this position is estimated to be around £30,000 - £40,000 per annum, depending on experience and qualifications. As a Band 4 Senior Ward Discharge Coordinator, you will be based onsite at the NNUH, working closely with the wider Integrated Discharge Team to facilitate smooth patient discharges.
