Social Care Practitioner Level One
Social Care
Adult Care and Community Wellbeing
Social Care Qualified - Adult

We are pleased to present an exciting opportunity within our Learning Disability Team. This position requires a qualified professional in either Social Work or Nursing, with a current registration from the relevant governing body.

Role Overview

In this role, you will be part of a multi-disciplinary team dedicated to supporting individuals with learning disabilities who have specific care and support requirements.

The successful candidates will be tasked with managing Community Deprivation of Liberty (DoLS) applications to the Court of Protection (COP).

Essential knowledge of The Care Act 2014, statutory responsibilities, and familiarity with the COP application process, DoLS legislation, and related documentation is crucial. Experience with the Mental Capacity Act, conducting mental capacity assessments, and applying best interest principles is also required.

You will operate autonomously while managing a diverse and complex caseload. Collaboration with our Learning Disability case management teams, legal departments, service providers, healthcare professionals, families, and advocacy services will be essential.

Timely execution of tasks is vital, as you will need to prioritize and coordinate with colleagues to ensure the necessary evidence for DoLS applications is collected promptly.

This role involves travel across the region, necessitating access to a vehicle for effective transportation to fulfill job responsibilities. Mileage claims will be reimbursed for travel from the office to appointments.

While this position is primarily focused on DoLS responsibilities, there may be occasions where the practitioner will need to engage in other Adult Care assessments or respond to emergencies or safeguarding issues. Flexibility in the role is important to meet the evolving needs of the service.

Key Responsibilities:

  • Manage a designated caseload of clients within the Learning Disability service, whether residing in supported living arrangements or their own homes.
  • Evaluate existing DoLS orders alongside key workers to assess current care and support, restrictions, and the individual's capacity to understand and consent to their care.
  • Collaborate with stakeholders to determine if the care provided is in the individual's best interests and is the least restrictive option available.
  • Complete necessary DoLS application documentation, coordinating with providers to gather signed care plans, risk assessments, and other relevant information.
  • Maintain accurate case notes and records, prepare reports as needed, and provide evidence in court regarding care-related matters.
  • Engage with families to explain the DoLS process, support their understanding of the role of the Rule 1.2 representative, and facilitate referrals to independent advocacy services when appropriate.
  • Adhere to legal frameworks and foster effective working relationships with legal services.

Our core office hours are structured to support work-life balance, with a blend of remote and office-based work as required to meet service demands.

Candidate Profile

We value our teams and the services we provide; therefore, it is crucial to recruit individuals who can deliver exceptional support to our teams and service users.

Applicants must possess either a Social Work or Nursing qualification and demonstrate current registration with the appropriate professional body.

Ideal candidates will exhibit the following qualities:

  • A proactive attitude, self-motivation, enthusiasm, and resilience.
  • Respect for individuals, ensuring dignity in all interactions.
  • Strong communication skills, with the ability to understand both verbal and non-verbal cues.
  • Creative problem-solving abilities.
  • Capability to perform effectively in high-pressure situations.

Team Overview

The Learning Disability team is committed to excellence in service delivery and is passionate about supporting individuals who access our specialized services.

Our dedicated team brings a wealth of knowledge and skills, fostering a supportive environment where each member contributes their unique strengths.

We recognize and celebrate our team's achievements, ensuring that practitioners feel valued and supported. We offer:

  • A competitive salary and generous pension scheme.
