Social Care Practitioner Level One

Social Care
Adult Care and Community Wellbeing
Permanent
Full-time
We are pleased to present an exciting opportunity within our Learning Disability Team. This position requires a valid qualification in Social Work or Nursing, along with current registration with the relevant professional governing body.
Role Overview
In this role, you will collaborate within a multi-disciplinary team to assist individuals with learning disabilities who require care and support.
The selected candidates will be tasked with managing Community Deprivation of Liberty (DoL's) applications to the Court of Protection (COP).
A strong understanding of The Care Act 2014, our statutory responsibilities, and familiarity with the COP application process, DoLS legislation, and related documentation is essential. Experience with the Mental Capacity Act, conducting mental capacity assessments, and adhering to best interest principles is also required.
You will be expected to operate independently while managing a diverse and complex caseload. Collaboration with our Learning Disability case management teams, legal departments, service providers, healthcare professionals, families, and advocacy services will be crucial.
Adherence to strict timelines is necessary, and you will work alongside colleagues to ensure that evidence for DoL's is collected promptly.
The role involves travel across various locations, and you must have access to a vehicle to fulfill the requirements of the position effectively. Mileage claims will be reimbursed for travel from the office to appointments.
Although this role is primarily focused on DoLS work, there may be occasions when the practitioner will need to engage in other duties, such as conducting new Adult Care assessments or responding to emergencies or safeguarding issues. Flexibility in this regard is essential.
Key Responsibilities:
- Manage a designated caseload of clients within the Learning Disability service, residing in community-supported living arrangements or their own homes.
- Review existing DoLS orders in collaboration with key workers to assess current care and support, restrictions, and the individual's capacity to understand and consent to their care.
- Engage with relevant stakeholders to determine whether the care provided aligns with the individual's best interests and is the least restrictive option available.
- Complete necessary DoLS application documentation, liaising with providers to gather required care plans, risk assessments, and medical evidence.
- Maintain accurate case notes and records, prepare reports as needed, and provide evidence in court regarding care-related matters.
- Support families in understanding the DoLS process and the role of the Rule 1.2 representative, facilitating referrals to independent advocacy services when appropriate.
- Work within established timeframes while understanding the legal framework and fostering effective relationships with legal services.
Candidate Profile
We value our teams and the services we provide; thus, it is vital to recruit individuals who can deliver the best support to our teams and service users.
Candidates must possess a Social Work or Nursing qualification and demonstrate current registration with the appropriate regulatory body. The nursing qualification does not need to be specific to Learning Disabilities.
Ideal candidates will exhibit the following skills and qualities:
- A proactive approach with self-motivation, enthusiasm, and resilience.
- Respect for individuals, ensuring dignity in all interactions.
- Strong communication skills, with the ability to understand both verbal and non-verbal cues and demonstrate professional curiosity.
- Creative problem-solving abilities.
- Capability to perform effectively in high-pressure or emergency situations.
The Learning Disability team is dedicated to delivering high-quality services and takes pride in their work. Our team members bring diverse skills and qualities, creating a supportive and nurturing environment.
Our Offer
We recognize and celebrate our team's achievements. Practitioners are valued and supported through regular meetings and professional development opportunities.
In addition to a competitive salary, we offer:
- A comprehensive pension scheme
- Reimbursement for professional registration fees
- Opportunities for career advancement, including training for Practice Educator/AMHP roles
- Generous annual leave entitlement
