Social Care Practitioner Level One
Adult Care and Community Wellbeing
Permanent Position
Full-time
We are excited to present an opportunity within our Learning Disability Team. This role necessitates a valid qualification in Social Work or Nursing, along with current registration with the relevant professional governing body.
Role Overview
As a practitioner, you will engage in a multi-disciplinary approach to assist individuals with learning disabilities who require care and support.
The successful candidates will be tasked with managing Community Deprivation of Liberty (DoLS) applications to the Court of Protection (COP).
Essential knowledge of The Care Act 2014, statutory responsibilities, and familiarity with the COP application process, DoLS legislation, and associated documentation is required. Experience with the Mental Capacity Act, conducting mental capacity assessments, and adhering to best interest principles is also necessary.
You will operate autonomously while managing a complex caseload and will collaborate with our Learning Disability case management teams, legal departments, service providers, healthcare professionals, families, and advocacy services.
Timely execution of tasks is crucial, and you will need to prioritize effectively while ensuring that evidence for DoLS is collected within stipulated timeframes.
This role involves travel across the region, and you must possess a motor vehicle to fulfill the travel requirements effectively. Mileage claims will be reimbursed for travel from the office to appointments.
While this position focuses on DoLS responsibilities, flexibility is expected, as you may also engage in other duties such as conducting new Adult Care assessments or responding to emergencies or safeguarding issues.
Key Responsibilities:
- Manage a designated caseload of clients within the LD service, residing in community-supported living arrangements or their own homes.
- Review existing DoLS orders alongside key workers to assess current care provisions, restrictions, and the individual's capacity to understand and consent to their care.
- Collaborate with stakeholders to determine if the care remains in the individual's best interests and is the least restrictive option available.
- Complete DoLS application documentation, liaising with providers to gather necessary care plans, risk assessments, and medical evidence.
- Maintain accurate case notes and records, prepare reports as needed, and provide evidence in court regarding care-related matters.
- Educate families about the DoLS process, support them in understanding the role of the Rule 1.2 representative, and facilitate referrals to independent advocacy services when appropriate.
- Work within established timeframes, understanding the legal framework while fostering effective relationships with legal services.
About You
We value our teams and the services we provide; therefore, it is essential to recruit individuals who can deliver the best support to our teams and service users.
Applicants must hold a valid qualification in either Social Work or Nursing and demonstrate current registration with the appropriate professional body.
Ideal candidates will possess the following skills and attributes:
- A proactive attitude, self-motivation, enthusiasm, and resilience.
- Respect for individuals, ensuring dignity in all interactions.
- Exceptional communication skills, with the ability to understand both verbal and non-verbal cues.
- Creative problem-solving abilities.
- Capability to perform well under pressure or in emergency situations.
Team Overview
The Learning Disability team is dedicated to providing high-quality service and takes pride in their work. Each member contributes unique skills and qualities, fostering a supportive and nurturing environment.
Our Offer
We celebrate our team's achievements and ensure that practitioners feel valued and supported. Alongside a competitive salary, we offer:
- A comprehensive pension scheme
- Reimbursement for professional registration fees
- Opportunities for career advancement, including training for Practice Educator/AMHP roles
- Generous annual leave entitlement
