Position Overview

As a Patient Transition Coordinator, you will oversee the organized discharge processes for both straightforward and intricate cases, actively working to mitigate delays in patient discharge and flow, ensuring that individuals receive the necessary care in a timely manner.

This role offers a unique chance to engage with external organizations, fostering robust partnerships to ensure uninterrupted care for our patients.

Main Responsibilities

  • Direct the coordinated discharge support within designated wards, including complex cases that may lead to Delayed Transfers of Care (DTOC) or those deemed Medically Ready for Discharge (MRD). Collaborate closely with the discharge coordinator to facilitate uncomplicated discharges.
  • Engage with external agencies and organizations to streamline discharge processes.
  • Establish and nurture effective relationships with relevant professional partners.
  • Organize and lead meetings that involve healthcare professionals, patients, and their families.
  • Deliver comprehensive information regarding all discharge arrangements and services to patients and their families/carers, both verbally and in written form.
  • Evaluate, monitor, and report on patient length of stay, delayed transfers of care, and related practice issues.
  • Assess and report on residency criteria and delays as initiated by the multidisciplinary team during board rounds, ensuring compliance with reporting requirements.
  • Proactively support and lead discharge initiatives during critical periods, such as Business Continuity episodes.
  • Manage, develop, and support the Discharge Coordinator team.
  • Assist in the collection and auditing of discharge data, being prepared to account for: - Additional bed-days beyond Estimated Date of Discharge (EDD), carefully noting reasons for any delayed discharges. - Repeated extensions of EDD. - Instances of 'failed discharge' or readmission within 48 hours for patients with the same condition.
