Job Overview

**Job Summary**

We are seeking a highly skilled and experienced Healthcare Science Coordinator to join our team at University Hospitals Sussex NHS Foundation Trust. As a key member of our Cardiology and Cardiac Surgery department, you will play a vital role in ensuring the efficient flow of patients through our wards, via elective and emergency admissions, as well as supporting the productivity and efficiency within our Cardiac Catheter Laboratory and Cardiac Theatres.

Main Responsibilities

**Communication**

  • Communicate complex information effectively to staff, patients, and their relatives.
  • Communicate effectively with other members of the multi-disciplinary team, including cardiologists, cardiac surgeons, nursing and allied health teams, and operational managers.
  • Communicate effectively with the site management and discharge team to ensure the most effective use of beds and optimize patient pathways.
  • Communicate with patients and relatives using empathetic skills, particularly in high-stress situations.
  • Communicate at daily operational meetings to ensure the status of the service is shared and any concerns or challenges are addressed.
  • Liaise with the multi-disciplinary team to ensure a high-quality, patient-centered service is provided in a safe clinical environment.
  • Carry a DECT phone to facilitate effective communication during working hours.
  • Confidently deal with distressing or emotional situations.
  • Ensure confidentiality and adherence to the Data Protection Act in relation to own practice.
  • Coordinate and communicate effectively with the emergency department to ensure cardiac patients are admitted within expected timeframes.
  • Maintain a good working relationship with the local cardiac network and relevant service providers.
  • Inform relevant managers of any untoward incidents and ensure appropriate investigation, corrective action, and documentation take place.
  • Utilize all lines of communication to ensure patients are discharged safely and in a timely manner.

**Service Delivery and Improvement**

  • Contribute to the development of clinical standards and maintain a knowledge base on current cardiology and cardiac surgical treatments and medications.
  • Contribute to the operational and strategic planning of the service.
  • Work collaboratively with the team to implement National and Local policies, protocols, and standards.
  • Effectively manage beds within the Cardiac Care Centre to ensure optimal use of available beds.
  • Facilitate and actively participate in the auditing of policies and procedures.
  • Ensure the principles of lean working are embedded into the Unit culture and reflected in practice.
  • Work collaboratively with the cardiac directorate clinical and management teams to develop expert, person-centered practice.
  • Engage in audits that assess the effectiveness of the service to ensure quality assurance.
  • Utilize research to enhance and implement research-based practice.
  • Engage with the DATIX incident reporting system to ensure that any accidents/actions/incidents are escalated appropriately.
  • Work with infection control teams, Cardiac teams, and facilities to ensure infection control procedures and cleanliness are maintained within the department.
  • Work with PALS and complaints team to ensure areas of concern are addressed in a timely manner.
  • Proactively participate in the evaluation of the service, collecting, analyzing, and presenting relevant data to identify, recommend, implement, and review all aspects of the service.
  • Help to develop and maintain the patient pathway supporting patients and relatives through their period of care.
  • Assist with the identification/implementation of service improvements and develop practice in line with the modernization agenda, Trust, directorate, and departmental objectives.

**Admissions, Discharges, Repatriations, and Transfers**

  • Manage the admission of acute emergency transfers from other hospitals, making appropriate changes to schedules at short notice, and communicating to all relevant staff.
  • Ensure adherence to same-sex accommodation when allocating patients on admission and at all times.
  • Provide a direct link between patients, carers, their GP, medical staff, referring hospitals, and appropriate Consultants.
  • Support the discharge of patients before midday and utilize the discharge lounge where possible.
  • Arrange transport for patients in line with the Patient Access/transport policy to ensure timely admission or discharge.
  • Provide a link between the clinical areas and Medical Staff concerning the availability of beds and the causes of cancellations.
  • Facilitate the repatriation of patients in conjunction with the wards/site teams on each hospital site and ensure the relevant online repatriation form is completed and sent.
  • Manage all emergency and urgent referrals from within the trust, working with the consultant of the week on duty to ensure clinical prioritization and timely treatment for these patients.
  • Coordinate the safe and efficient transfer of patients from DGH's for all cardiac treatment/s in line with the 'transfer' policy.
  • Ensure effective procedures are in place to ensure a seamless discharge from the acute setting to Primary Care facilities/services in the community.
  • Manage activity in line with contractual agreements, working closely with the general manager for the Cardiac Directorate in terms of performance management for all access targets.
  • Support the Patient Access Team and operational managers in reducing waiting times for elective procedures, ensuring the service is as efficient and productive as can be with excellent organizational skills.

**Person Specification**

**Essential Criteria**

  • NMC registered.
  • Cardiology/Cardiac Surgery modules.
  • Cardiac experience, of which a substantial time period is at Band 6 level.
  • Evidence of working in a Cardiac Coordinator role.

**Desirable Criteria**

  • Substantial post-registration experience.
  • Leadership course or equivalent.
  • Masters Module.
  • Experience of using Patient First Improvement System.
