Acute Medicine Physician

We are seeking a skilled Acute Medicine Physician to join our team at Central London Community Healthcare NHS Trust. As a key member of our multidisciplinary healthcare team, you will be responsible for providing medical care to patients in our bedded rehabilitation units.
Key Responsibilities- Provide comprehensive medical assessments and reviews to patients on the units, including new admissions and stable or deteriorating patients.
- Coordinate medical care, including liaison with community and hospital colleagues to ensure continuation of pre-existing plans and escalation of cases to acute care settings when necessary.
- Document medical records and communications between providers in line with professional guidelines and Trust policies.
- Experience in acute medicine and rehabilitation
- Strong communication and teamwork skills
- Ability to work in a fast-paced environment
