About the Role

We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our team at Midlands Partnership NHS Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care and support to individuals with mental health needs in the community.

Key Responsibilities
  • Assess and manage complex caseloads, including specialist assessments, care planning, and evaluation of individual treatment plans.
  • Participate in the setting of quality standards, auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Deliver a range of interventions, including pharmacological and psychologically informed interventions, to support individuals to meet their agreed goals.
  • Complete and contribute to robust risk assessments and risk management plans, promoting positive risk taking to improve quality of life and maximize occupational performance/independence.
  • Maintain an effective reporting system, observing and reporting verbally and in writing on patient conditions.
  • Utilize agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.

Person Specification

We are looking for a candidate who meets the following criteria:

  • Registered Mental Health Nurse on NMC Register
  • Conforms to NMC requirements
  • Experience of working with people experiencing mental health-related difficulties
  • Experience of working with people who have difficulty engaging with services and have complex mental health needs
