Job Title: Community Support Worker

We are seeking a highly motivated individual to support people with dementia and their carers in identifying their needs and providing information and guidance to maintain their independence.

About the Role:
  • Develop and maintain relationships with local contacts, networking with healthcare professionals and marketing our services to support individuals affected by dementia.
  • Collaborate with partner organizations to ensure seamless and consistent support for people with dementia and their carers.
  • Provide expert guidance and support to individuals affected by dementia, promoting their independence and well-being.
About the Partnership:

Our partnership with Cadent Gas offers a unique opportunity to provide clear and up-to-date advice about energy efficiency, the dangers of carbon monoxide, and the benefits of the Priority Services Register to people affected by dementia.

About the Community:

This role will focus on supporting the South Asian community within our service area, requiring a strong understanding of the community's needs and a commitment to providing culturally sensitive support.

About the Ideal Candidate:
  • A highly motivated individual with a strong understanding of dementia and the needs of people affected by dementia.
  • Excellent communication skills to meet the diverse needs of our community and represent their needs to statutory and other voluntary agencies.
  • Sound knowledge of IT systems to record data, write reports, and communicate effectively.
