Job Title: Community Support Worker

We are seeking a highly motivated individual to support people with dementia and their carers in identifying their needs and providing information and guidance to maintain their independence.

About the Role:
  • Provide expert support and guidance to individuals affected by dementia, empowering them to make informed decisions about their care and well-being.
  • Develop and maintain relationships with local health and care professionals, ensuring seamless and consistent support for individuals affected by dementia.
  • Collaborate with partner organisations to develop networks and pathways that promote independence and quality of life for people with dementia.
About the Partnership:

As a partner of Alzheimer's Society, you will have the opportunity to work with a leading charity that provides clear and up-to-date advice about energy efficiency, the dangers of carbon monoxide, and the benefits of the Priority Services Register to people affected by dementia.

About the Community:

This role will focus on supporting the South Asian community within Newham, Enfield, and Barnet, requiring a strong understanding of the community's needs and cultural nuances.

About the Ideal Candidate:
  • A highly motivated individual with a strong understanding of dementia and its impact on individuals and communities.
  • Fluent in South Asian languages, particularly Urdu, and potentially Hindi.
  • Excellent communication and interpersonal skills to build strong relationships with diverse stakeholders.
  • Strong knowledge of IT systems to record data, write reports, and communicate effectively.
