About the Role

We are seeking an experienced and skilled Palliative Care Specialist Nurse to join our team at Oxford University Hospitals NHS Foundation Trust. As a key member of our Community Palliative Care Team, you will play a vital role in delivering high-quality patient care and supporting the development of our nursing staff.

Key Responsibilities
  • Provide expert clinical assessment and judgement to patients with complex palliative care needs.
  • Work autonomously and as part of a multi-disciplinary team to deliver patient-centered care.
  • Develop and implement individualized care plans, taking into account the patient's physical, emotional, and social needs.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Contribute to the development and implementation of clinical policies and guidelines, ensuring best practice in palliative care.
  • Provide education and training to nursing staff, promoting excellence in palliative care practice.
Requirements
  • Graduate with 1st level registration as a nurse.
  • Post-registration qualification or equivalent experience in palliative care.
  • Experience teaching and assessing in clinical practice.
  • Demonstrable expertise in end-of-life or palliative care nursing.
  • Ability to discuss and demonstrate evidence-based practice.
  • Management role experience.
  • Knowledge and experience of change management.
  • Understanding of clinical governance and audit.
  • Evidence of ability to work across organizational boundaries.
