About the Role

The Palliative Care service at Oxford University Hospitals NHS Foundation Trust is seeking a highly skilled and experienced Palliative Care Specialist Nurse to join our dynamic multi-disciplinary team. As a Senior Specialist Nurse, you will play a key role in delivering specialist palliative care services to patients in Oxfordshire and South Northants.

Main Responsibilities

  • Provide expert clinical assessment and judgement to patients with palliative care needs
  • Work autonomously and as part of a multi-disciplinary team to deliver high-quality patient care
  • Develop and implement individualized care plans to meet the needs of patients and their families
  • Provide education and training to staff on palliative care principles and practices
  • Contribute to the development of palliative care services and policies within the Trust

About the Team

The Community Palliative Care Team is a dynamic and supportive team that provides specialist palliative care services to patients in their own homes, outpatient clinics, and through a telephone advice line. As a Senior Specialist Nurse, you will be part of a team that is passionate about delivering high-quality patient care and making a positive impact on the lives of patients and their families.
