Position Overview

The Palliative Care division is dedicated to providing expert palliative care and guidance to the residents of Oxfordshire and South Northants, in collaboration with local hospice organizations.

The Community Palliative Care Team operates a comprehensive service throughout the week, necessitating flexibility in working hours.

We seek individuals who exhibit outstanding communication abilities and possess strong leadership skills, capable of functioning as independent practitioners.

Experience in Specialist Palliative Care and familiarity with community health services are crucial.

Proficient history-taking and assessment capabilities are mandatory, and the role may require undertaking non-medical prescribing if not already qualified.

Key Responsibilities

The Community Palliative Care Team is a vibrant multi-disciplinary group that delivers specialized palliative care services across Oxfordshire and South Northants. This encompasses home visits, outpatient consultations, and a dedicated telephone support line. A valid driving license is essential, along with the ability to travel throughout the designated areas.

The team provides end-of-life care, symptom management, and training in communication skills for staff at all levels. A strong commitment to education and professional development is therefore vital.

As a lead nurse within the team, the successful candidate will be expected to demonstrate expert clinical assessment and judgment focused on patient needs. The role also involves supporting team members in their professional growth and fostering collaborative multi-disciplinary teamwork.

Job Responsibilities

Senior Palliative Care Nurses will be expected to:

  • Utilize their specialized knowledge and clinical judgment to determine the need for patient referrals and engage in discussions regarding potential diagnoses with the broader multi-disciplinary team.
  • Implement treatment plans, which may involve advising on medication management or referring patients to appropriate specialists.
  • Where qualified and registered, prescribe medications for symptom management.
  • Conduct comprehensive assessments of individuals, families, and communities using various methods, including advanced history-taking and specialized health assessments.
  • Ensure continuity of care for patients.
  • Evaluate treatment effectiveness in collaboration with patients and/or caregivers, making necessary adjustments.
  • Work independently within a larger healthcare team and as a solo practitioner.
  • Collaborate across professional and organizational boundaries, actively developing partnerships to enhance health outcomes and healthcare delivery.

Qualifications & Experience

Essential:

  • Registered nurse with first-level registration.
  • Post-registration qualifications or equivalent experience.
