We are seeking a highly skilled Specialist Nurse to join our Community Palliative Care Team. The successful candidate will have excellent communication skills and strong leadership qualities, with a proven track record in palliative or end of life care.

The Community Palliative Care Team provides specialist palliative care services to Oxfordshire and South Northamptonshire, including home visiting, outpatient clinics, and telephone advice. We offer a 7-day service, including a Rapid Intervention in Palliative and End of Life care project, which aims to enable patients to remain at home for the end of their life.

The successful candidate will be responsible for assessing individuals, families, and populations holistically, using a range of different methods, and deciding on and carrying out treatment, which may include advice on the prescribing of medicines or referring patients to an appropriate specialist.

The Specialist Nurse will work autonomously within a wider healthcare team and as a lone practitioner, working across professional, organisational, and system boundaries to influence and improve health outcomes and healthcare delivery systems.

The successful candidate will be able to plan, prioritise, and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use.

Key Responsibilities:

  4. Assess and evaluate the effectiveness of treatment and care provided and make changes as needed.
  5. Work autonomously within a wider healthcare team and as a lone practitioner.
  6. Work across professional, organisational, and system boundaries to influence and improve health outcomes and healthcare delivery systems.
  7. Plan, prioritise, and manage complete episodes of care, working in partnership with others.
  8. Use professional judgement in managing complex and unpredictable care events.
  9. Promote a high standard of specialist nursing care.
  10. Maintain adequate patient documentation to NMC requirements.
