Loneliness and Social Isolation Specialist

**Job overview**
To contribute to reducing loneliness and isolation across the Black Country with a particular focus on those accessing services at BCHFT. To support a caseload of people with mental health and other health conditions to access the Lets talk service activities and provide one to one support.
The post holder will play a key role in identifying and supporting people across the Black Country and will support with service information, developing support groups for, signposting to additional support such as health & well-being and to help integrate into Community services.
**Main duties of the job**
- Supporting communities to build social networks
- Supporting volunteers to co-ordinate a range of activities
- Developing face to face support groups
- Provide one to one support to help those socially isolated
- Developing networks with key partners to build capacity in social networks to reduce loneliness
- Promote the service to the public, partners and clinical teams to highlight the project and source referrals

**Salary**: £28,407.00-£34,581.00 per year
