**Complex Emotional Needs (CEN) Recovery Practitioner ** will be integrated in to the Complex Emotional Needs Hub working alongside clinical colleagues. Supporting a caseload of individuals who have complex emotional needs or may have a diagnosis of personality disorder. The focus of the role is to practically support individuals to identify and achieve their recovery goals, to build and maintain a meaningful future. This can include promoting healthy lifestyles, building confidence and resilience. Offering information, guidance and practical support to access local community resources. Providing both 1:1 and small group recovery focused interventions.
**Main duties of the job**:
This post will sit within the Employment and Recovery service line, with the CEN Recovery Practitioners integrating with the Complex Emotional Needs Hub.* The post holder will be required to utilise a range of skills to support the individual to achieve their recovery goals this may include goals related to mental, physical health, social, educational and employment aspirations and outcomes. *The post holder will be expected to link in when appropriate to the individuals needs with the other Employment and Recovery services including Individual Placement and Support (IPS) employment services, Thrive, Employment Retention services, and specialist services including the Let’s Talk. *The post holder will support the individual and their carer in close collaboration with the clinical care team.*The post holder will be expected to liaise with multiply internal and external partners in both the statutory and voluntary sector to support the individual in achieving their identified recovery goal. Flexibility and adaptability is required due to the unpredictability of the environment and resource availability under which work will need to be carried out.*The post holder will work to the Trust’s lone worker policy and local Safe and Well protocol. *The post holder will ensure that they have a fully charged mobile phone in working order so that communications can be achieved at all times.

**Detailed job description and main responsibilities**:
*To support a caseload of individuals accessing the Complex Emotional Needs Hub, some of whom will have multiple health conditions barriers.
*At times being exposed to distressing and emotional circumstances, experiencing unpleasant working conditions or having to support individuals who may have barriers to understanding.
*Sharing and receiving highly complex, sensitive or contentious information.
*To liaise closely with the Complex Emotional Needs Hub in managing risk or relapse in order to safeguard individuals and relevant others when potential risk of relapse or harm may be evident.
*To actively participate in multidisciplinary team meetings, reviews, professionals meetings, liaising with other professionals to ensure a seamless service is provided.
*To liaise and communicate regularly with the referrer and other relevant healthcare staff to ensure that all parties are kept informed and co-operate in helping the individual achieve their goals.
*To carry out holistic assessments, identify recovery goals and develop action plans in agreement with the individual.
*Providing personalised support to individuals, their families, and carers to take control of their wellbeing, live independently and improve their health outcomes.
*Developing trusting relationships by giving individuals time and focusing on ‘what matters to them’.
*To maintain detailed and accurate case notes, action plans and paperwork and upload these on to the clinical management information system within the allocated timeframe.
