**Job overview**:
An exciting opportunity has arisen within the liver unit at the QEHB for a Patient Pathway Co-ordinator to join the team in a fixed term 12 month position.

The Liver and Hepatobiliary Unit is the largest specialist Liver Unit in Europe. Liver Unit activity includes management of patients with acute and chronic liver diseases including those who will undergo liver transplantation and HPB cancers. The unit has busy inpatient and outpatient activity. The hepatology team work together with HPB surgical colleagues, providing the largest liver transplant program in the UK. The pathway coordinator works as part of a multi-disciplinary team to ensure the entire patient pathway is managed smoothly. You will learn and develop on the job, though we would expect some knowledge of the NHS and its systems, making this an excellent opportunity for someone with at least 12 months NHS experience. The liver unit is a large organisation which requires successful applicants to be versatile and flexibility in order to adapt to the needs of a very busy service.

**Main duties, tasks & skills required**:
In order to be successful in this role you will be:
focused on providing a high level of service to patients and healthcare professionals

able to follow procedures and guidelines in obtaining required imaging / histology and relevant correspondence from tertiary providers

Work autonomously to facilitate MDT listing after clinicians triage

Work alongside the waiting list co-ordinator to provide support for The Cancer 62 Day PTL (Patient Tracking List) data collection

Be able to communicate clearly and confidently with internal and external contacts as well as patients and their relatives and carers which includes the medical and surgical division and to support peers.

Previous experience as a patient pathway co-ordinator is advantageous but not essential.

The successful applicant would receive relevant training if needed
