We are looking for an organized, motivated, enthusiastic person to join our team and work collaboratively with the multi-disciplinary team, striving to achieve the highest standard of maternity care.

As our maternity discharge Co-Ordinator you are pivotal in the overall experience of clients leaving our trust. You will be responsible for facilitating the smooth and timely discharge of mum and baby from hospital to community care. With appropriate training we will support you to develop your knowledge and understanding of the needs of the newborn baby and be able to give practical support to mothers. We will upskill you on all our digital platforms so that you can be equipped to perform the administrative duties of the role.

We will equip you to be able to advise and ensure mothers have all the relevant information and contacts they require prior to discharge.

You will become instrumental in supporting our education program by upskilling the junior work force to develop their knowledge and understanding of the discharge management including all requirements within our digital platforms, sending of birth notifications, GP liaison & the discharge process including managing patient expectations.

Maintain momentum of discharge planning through supporting and working in partnership with other members of the Multi-Disciplinary Team (MDT) to identify patients early and manage expectations of the discharge process.

Act as a first point of contact for both internal and external enquiries relating to discharges.

Act as a failsafe officer to ensure all communications with community partners, in & out of area have received the discharge information so ongoing community care can be provided.

Co-ordinate the team to minimise and reduce delays in discharge.

To ensure all risk assessments have been completed on our digital platforms.

To be part of the ward handovers identifying potential early discharges

To liaise with pharmacy to get take away medication (TTA) to the ward in a timely manner.

Ensure accurate and timely documentation is completed.

Will co-ordinate and promote the use of the discharge lounge at times of high acuity.

Conduct audits within the clinical area relating to patient experience.

Support the Ward Manager with implementing changes to improve patient experience.
