**Gateway Co-ordinator**

**12 months maternity cover**

This may include providing information, signposting or referral to other providers and agencies, or connection to a service or activity.

Contributing to the delivery and take-up of The CHAOS Group’s core and developing services such as:

- The Step into Wellness programme
- Accepting clients for the Hospital to Home Service Co-Ordinators
- Accepting clients from Accident & Emergency for the Hospital to Home Service CoOrdinators
- Triaging new referrals for Mental Health Peer Support Workers
- Information and Guidance
- Signposting to both internal and external sources within statutory and charitable sectors

Development, coordination, and reporting of the Kernow Gateway’s work. This includes working with our partner agencies to improve the service of the Gateway to Cornwall’s residents; and providing guidance, reassurance and decision-making support to Kernow Gateway.

A blended and flexible approach to work is required, working from home and office to support a 7-day service.

Please Note: your core hours will be in negotiation with you - but you will be expected to be flexible in contributing to the 7 day a week service.

**Main Duties and Responsibilities**

1. Hold effective person-centred, conversations with clients, following the organisation's guidance, to explore their needs and identify a range of appropriate options for them. Checking with them to ensure you have provided sufficient accurate, clear and relevant information to support them in accessing their chosen options.

2. Work with the Age UK communications lead to share stories and events on social media and generally spread the word about the difference the Gateway is making; raising awareness of their work and ours.

3. Updating IT systems in a timely manner to ensure all data is complete and accurate, and adds value to the service delivery.

4. Liaise with and update service & administration teams and referrers to ensure they are aware of actions taken and required, with full attention to confidentiality & data protection at all times.

5. Maintain own knowledge and understanding of the services and activities provided by the organisation and by other agencies and community groups.

6. To work as part of the wider team, contributing to the overall success of our services, projects and outcomes thereby helping ensure all those who interact with us experience excellent person-centred responses.

7. Planning and coordination of the Gateway’s operational requirements.

8. Coaching and supporting staff to be fully competent in their role.

9. Actively participate by working with colleagues in other areas of the wider organisation.

10. Accepting clients from the Gateway Helpline or Accident & Emergency Social Prescribers, inputting data on Charity Log to pass to the Hospital to Home Co-Ordinators

11. Triaging new referrals received from Helpline or Community Mental Health and inputting data on Charity Log to pass onto the appropriate Mental Health Peer Support Worker.

12. Carrying out any other duties as required; which are consistent with the duties and responsibilities of the post.

13. Adhere to all organisational policies and processes, including arrangements for confidentiality agreements and contacts, safeguarding, and GDPR.

**Key Contacts and Relationships**

The post holder will be responsible to the Gateway Lead. Other key contacts will be the Chief Executive, and senior managers. External relationships will be maintained with service and referral teams.

**Office Base**

CHAOS, 2-3 St Clement Vean, Truro. TR1 1RN. There will also be opportunity for blended working from home.

**Hours**

Full time available to cover an 8:00am - 8pm service provision. As the role develops to support a 7-day service and the work pattern evolves to include out of hours, weekends and/or evenings, the post-holder is required to demonstrate reasonable flexibility.
