An opportunity has arisen to join the Older Peoples Service & Transfer of Care Hub team at Colchester Hospital as a Transfer of Care Referrals Co-ordinator.

The post holder will be required to carry out a wide range of duties to support the Transfer of Care Hub (TOCH) in the delivery of a high quality care and effective discharge planning from the acute hospital.

The main focus of the Referrals Co-ordinator is to assess patients that have been referred into the TOCH within the first 24 hours to identify their care needs, with the aim to discharge as many patient’s with Pathway 0/1 on the same day as referred
- To prioritise and assess new referrals into the TOCH, including Early Identification of Complex Discharges, within 24 hour period to recognise the level of care required.
- To liaise with the ward MDT, wider services within the Transfer of care hub, the patients and their families to appropriately identify and signpost a suitable pathway, with the aim to discharge home the same day or progress onto allocation of a Case Manager.
- The post holder would be expected to educate patients, families and ward staff regarding eligibility for care and to understand for their own health and social care needs, whilst promoting the Home First Ethos
