The trust is in early stages of designing a new Hospital at Home model that will support patients to access acute hospital care in their own homes, utilising digital technology and supported by a multidisciplinary team. The programme will provide face to face and virtual care, working in the acute trust as well as the community. - To be responsible for the management of the patient’s pathway, from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the
- To provide effective administrative support to ensure smooth facilitation and co-ordination of multi-disciplinary team meetings (MDTs) on behalf of MDT
- To maintain up to date knowledge of all targets and processes relating to the patient pathway process and to be a point of reference within the department of other members of the
- To assist the MDT leads in the development and refinement of all MDT processes

- To ensure that appropriate requests for investigations are managed in a timely If diagnostic delays occur or appear imminent, it is the responsibility of the post holder to escalate to the relevant Manager.
- To utilise negotiation skills effectively regarding, diagnostic testing and any other relevant procedures regarding the patient’s effective pathway
- To liaise with members of the individuals specialities MDT, and across the trust as a whole to ensure co-ordination of all elements of the patient This may include highlighting patient target dates etc. to ensure that investigations are carried out and results received in a timely manner.
- To ensure all information relating to the patient’s pathway clearly identifies all relevant target dates to aid other members of the MDT in allocating resources
- To inform the MDT lead of any potential delays or breaches of the patient pathway according to the Trust Escalation
- To identify bottlenecks/delays in the patient pathway and to follow the Trust escalation policy to raise awareness to the Service Managers as
- To track patients who have opted out of part of the pathway, and ensure that the timings for patient pathways are correctly calculated
- To agree with the MDT Lead and Clinical Nurse Specialist (where appropriate) a process of identifying new cases to be discussed at the multidisciplinary team
- To produce a list of new patients to be discussed at the MDT using pathology and radiology lists and as advised by members of the MDT and distribute as required to all members prior to the meeting, and after the meeting circulate a list of all patients discussed indicating main conclusion from the
