We are looking to appoint a Band 4 Pathway Co-ordinator to help underpin the patient journey and support the clinical team in delivering high quality patient care that can make a real difference to the patient experience. You will provide a professional comprehensive and efficient administrative service to the palliative care Lymphoedema teams at Sobell House and Katharine House, and supervise a Band 3 Assistant Pathway Co-ordinator.
You will be expected to demonstrate a professional, sensitive manner, and be experienced in greeting and handling calls to and from patients, relatives, clinical staff and beyond. You will be required to work flexibly, juggling shifting priorities in this key supporting role within the team and may be asked to cross-cover elsewhere within the admin team.
You will have a proven ability to work to a high degree of precision whether it is maintaining excellent communication systems, recording data onto iCare and EPR, or producing and reconciling monthly reports.
The successful applicant must have excellent communication and interpersonal skills, be capable of using initiative, have good I.T skills and be confident working independently and managing reports.
You will be based at Sobell House.

The post holder has responsibility for co-ordinating a professional, comprehensive and efficient administrative service, which is effectively delivered to all patients and members of the department. This will be through the delivery of a comprehensive administrative service to a specialty team.
To be the first point of contact on all administrative issues, and to provide efficient and comprehensive support to the Lymphoedema Team.
To facilitate communication and systems between Lymphoedema, other teams within palliative care and the Trust.
To keep accurate up-to-date records on iCare and EPR.
To support the team with the preparation of teaching sessions and educational materials as required.
To cross cover within the team for annual leave and sickness.
To carry out appraisals for any Assistant Pathway Co-ordinators within your team.
The post holder will be expected to have excellent IT skills, be capable of using his/her own initiative, take responsibility for organising the workload, and enjoy working independently as well as a team member.
To be able to manage their defined areas within the speciality and be responsible for prioritising work, developing staff, reviewing working practices to ensure the effective running of the department and effective communication structures are implemented.
