Supporting improved uptake rates of diabetic eye screening and reducing unwarranted variation in uptake by different population groups - to reduce health inequalities.

To be the first point of contact for GP practices and community groups, provide comprehensive advice and support for health care professionals and wider organisations to improve the uptake of diabetic eye screening.

To provide pro-active and outward/public facing support to the diabetic eye screening programme team in the improvement of screening uptake across the diabetic eye screening programme.

The post holder will work as part of a dynamic team in delivering an effective service, supporting programme managers and screening staff across the diabetic eye screening programme.

The post holder will focus on improving the uptake of diabetic eye screening across the programme geography.

**In particular, the post holder will**:

- Liaise directly with healthcare colleagues in Primary Care (GP practices and/or PCN’s), community groups and Place colleagues (ICB, independent sector and local authority) to improve the uptake of diabetic eye screening particularly by population groups vulnerable to non/low participation in screening
- Promote uptake of diabetic eye screening directly to the eligible population at health promotion events/webinars held across the programme geography
- Support eligible individuals to navigate through the screening pathway - particularly those who may require reasonable adjustments to reduce unnecessary barriers to access
- Use screening programme data to identify GP practices with low uptake rates and work directly with colleagues in these practices e.g. care co-ordinators/care navigators to support more individuals to take up the offer of screening
- Identify reasons for non-attendance across the screening programme and discuss with the programme team any changes that may help address these to support improved uptake rates
- Maintain an up to date and accurate recording system of work to evidence the impact of the post of Improving Screening Uptake Worker

**Key priorities include**:
1. Increasing uptake of individuals who have not attended for screening in 3 years.
- Work with the Programme Manager to identify and contact all eligible S08 individuals who have not attended for screening in 3 years and support the programme in meeting the acceptable/achievable thresholds for KPI DE4 by targeting interventions at persistent non-attenders
- Work collaboratively with PCNs/practices to ensure diabetics are correctly coded and accurate information is transferred via GP2DRS to the Diabetic Eye Screening Programme to the Single Collated Lists (SCL) held by the Diabetic Eye Screening
- Proactively contacting individuals who have either not responded or not attended diabetic eye screening appointments
- Targeting interventions for priority groups including pregnant individuals who require more frequent screening during pregnancy
- Proactive engagement with local prisons/secure settings to ensure screening is offered within relevant time scales and work towards reducing DNAs

3. Supporting individuals with a learning disability to access diabetic eye screening taking learning from the NHSE NW 2-year programme of work included below.
- Working with practices to identify individuals eligible for diabetic eye screening who are on the learning disability register and contacting individuals to support in accessing screening
- All individuals with a learning disability in target practices to receive a pre-appointment telephone call
- All individuals to be offered a reasonable adjustments plan and easy read materials
- To monitor the number of individuals with a learning disability who take up the offer of screening
- Develop an easy read patient satisfaction survey for individuals with a learning disability - this can be done in collaboration with other C&M and L&SC Improving diabetic eye Screening Uptake Officers
