Continuing Health Care Coordinator

A rare and exciting opportunity has arisen for a permanent, fulltime position of a Continuing Healthcare Coordinator in the Lead Professional Team. If you have a Social Work, Occupational Therapy or Nursing qualification and you can demonstrate a track record of working effectively with partners in applying the NHS Continuing Health Care Framework, we have an exciting opportunity for you.

About the role

In this exciting role the successful candidate will work alongside the current CHC Coordinator to bringing their own operational experience and knowledge of NHS Continuing Health Care to the team and further build their knowledge and expertise. It is an ideal opportunity for someone to balance a practice role with a practice leadership and development role.

As CHC Coordinator, you will support staff members to apply the framework and joint work the more complex cases alongside the adult care practitioner. Your role is also to ensure that guidance, training, resources and practice standards are in place ensuring that CHC activity is monitored using correct systems.

You will represent Lincolnshire Adult Care on panels, supporting disputes and in maintaining effective relationships with NHS Partners.

The successful candidate will be able to demonstrate a track record of working effectively with partners in applying the NHS Continuing Health Care Framework, coupled with:

  • Considerable post qualifying knowledge and experience of NHS Continuing Health Care and an understanding of the law as it relates to Adult Care practice.
  • A thorough understanding of the financial implications of continuing healthcare in relation to individuals and adult care,
  • The ability to inspire staff with a quality approach towards achieving appropriate outcomes for individuals and the council in the continuing healthcare process,
  • Be able to make decisions, and where appropriate action them, using your knowledge, judgment, and experience.
  • A proven ability to work in an environment where you effectively supporting staff with very complex high-risk cases and risk management associated with actions taken in relation to advice / support provided on continuing healthcare.
  • Ensuring that Adult Care views, practice and processes are appropriately raised and considered in any joint decision making.
