Position Overview

St Barnabas Hospice is seeking a dedicated Palliative Care Coordination Manager to join our compassionate team. This role is essential in supporting the Palliative Care Coordination Centre (PCCC) and ensuring seamless service delivery.

Working Hours:

30 hours per week, Monday to Friday.

Work Environment:

This position primarily operates remotely, with some initial training conducted in a designated office environment.

Employment Type:

Fixed-term contract for a duration of 6 months.

Compensation:

Annual salary of £24,048, pro rata, with additional enhancements for weekend and unsocial hours.

Key Responsibilities:

The PCCC serves as the primary access point for St Barnabas Community Services. In this role, you will collaborate with a team utilizing a telephone network and specialized software systems. Your responsibilities will include:

  • Engaging with NHS, voluntary, and private sector care providers to coordinate care packages for patients wishing to remain at home.
  • Maintaining accurate records and managing data within our bespoke computer systems.
  • Facilitating communication between various stakeholders to ensure effective service delivery.

Qualifications and Experience:

We welcome applications from individuals who possess:

  • A minimum of GCSE qualifications at grade C or above in Maths and English, or equivalent NVQ/life skills qualifications.
  • Proficiency in computer skills, ideally to CLAIT/ECDL standards or equivalent.
  • Demonstrated administrative capabilities and experience in a multi-functional office setting, particularly in coordinating meetings and events.
