We are seeking a highly skilled and compassionate Palliative Care Centre Coordinator to join our team at St Barnabas Hospice. As a key member of our team, you will play a vital role in ensuring that patients receive the highest level of care and support.

Key Responsibilities
  • Coordinate patient care packages with NHS, voluntary, and private sector care providers
  • Liaise with patients, families, and healthcare professionals to ensure seamless care delivery
  • Utilize a bespoke computer database and other IT applications to manage patient information and coordinate care
  • Work collaboratively with a multidisciplinary team to provide comprehensive care and support
Requirements
  • GCSE standard in Maths and English or equivalent
  • Computer literacy to CLAIT/ECDL standard or equivalent
  • Proven administration skills and experience working in a multi-functional office environment
  • Excellent communication and interpersonal skills
