Job Title: Crisis Resolution and Home Treatment Team Member

The Crisis Resolution and Home Treatment Team is seeking dedicated professionals to join their community-based service. Main Responsibilities:

  • Conduct comprehensive clinical assessments to identify the care needs of service users in acute mental health crisis.
  • Develop and implement evidence-based care plans to ensure identified needs are met.
  • Provide professional advice and support to other agencies and team members.

Key Duties:

  • Deliver high-standard clinical care through safe and effective assessment and treatment.
  • Work collaboratively across a 24/7 service, including night shifts.
  • Manage a shared case-load and ensure clinical practices meet the highest standards.
  • Develop and implement care plans, risk assessments, and reviews.
  • Protect individuals from harm and enable independent living within the community.
  • Promote effective working relationships with the multi-disciplinary team.
  • Maintain accurate clinical records and adhere to professional codes of conduct.
  • Participate in Clinical Governance activities and continuous professional development.

Requirements:

Qualifications

Essential:

  • RVN or RNLD or RN: 1st Level Registration (NMC) or equivalent.
  • ENB998 / FliPS or equivalent.
  • Currently Registered Nurse / Allied Health Professional.
  • Mentorship Module.
  • Evidence of specialized continued professional training.

Desirable:

  • Management/Leadership training.

Experience

Essential:

  • Relevant experience with adults with mental health needs.
  • Experience with severe and enduring mental illnesses.
  • Continuous professional development.

Skills & Competences

Essential:

  • Demonstrate leadership ability with a clinical team.

Special Requirements

Essential:

  • Ability to travel independently throughout the county.

Desirable:

  • Driving Licence.
