About the Role

We are seeking a highly skilled and compassionate Acute Care Practitioner to join our Crisis Resolution and Home Treatment Team at Lincolnshire Partnership NHS Foundation Trust.

Key Responsibilities
  • Conduct comprehensive clinical assessments to identify the care needs of service users presenting with acute mental health crises or symptoms of mental distress.
  • Develop and implement evidence-based care plans to ensure identified care needs are met, enabling service users to transition from crisis point to discharge.
  • Provide professional advice and support to other agencies and members of the multi-disciplinary team.
  • Work collaboratively with individuals, carers, and families to assess their mental health needs, recognize mental illness, and develop strategies to manage and change behavior.
  • Plan, implement, review, and improve interventions to meet people's identified needs and manage inherent risk.
  • Assess carers' and families' needs and develop, implement, and review programs of support.
  • Protect people from abuse, neglect, and harm.
  • Enable individuals to develop independent living skills and live in the community.
  • Maintain accurate and timely clinical records and coordinate and monitor those of the team.
  • Adhere to registered practitioners' codes of professional conduct and ethics, plus associated legislation.
  • Develop clinical practice having due regard to guidelines for Older People, for Mental Health, and other relevant frameworks.
  • Demonstrate empowering leadership skills within the team and seek opportunities to promote and develop the profession.
  • Participate in Clinical Governance activities, including induction, supervision, personal development review, health and safety, risk management, and audits.
  • Undertake specific project work or other duties as negotiated with the CRHT Team Co-ordinator/Manager.
  • Develop effective and supportive links with other health and social care colleagues and teams to improve the pathway of care for service users referred to CRHT.
  • Participate in all clinical governance and audit developments, including post-registration education, training, and continuing professional development.
Person Specification
  • Essential:
    • RVN or RNLD or RN: 1st Level Registration (NMC) or degree or equivalent.
    • ENB998 / FliPS or equivalent.
    • Currently Registered Nurse / Allied Health Professional.
    • Mentorship Module.
    • Evidence of specialized continued professional training in clinical practice.
  • Desirable:
    • Management/Leadership training.
