Physical Health Liaison Specialist

We are seeking a highly motivated and experienced individual to join our team as a Physical Health Liaison Practitioner for Adults with Learning Disabilities. This is an exciting opportunity to work in a specialist learning disability service and contribute to the development of new ways of working across an integrated care system.
Key Responsibilities- Provide physical healthcare support and liaison to professionals within the Learning Disability service and partner agencies, including GP practices and services in United Lincolnshire Hospital Trust (ULHT).
- Ensure a high standard of physical healthcare support is provided for service users with a Learning Disability whose complex physical healthcare needs cannot be met without the support of specialist Learning Disability services.
- Manage a small complex caseload of patients who have a learning disability, working closely with the ULHT safeguarding lead nurse to coordinate patient care and provide support to Acute liaison nurses and primary care colleagues.
- Support the validation of GP Quality Outcome Framework Learning Disability Registers annually, engaging with practices whose registers are not part of the validation process to understand if additional support is required.
- Deputise in the absence of the physical health liaison lead and provide management and clinical supervision to staff within the service, as well as provide support and training in respect of community and acute physical healthcare to the Learning Disability Service.
