About the Role

We are seeking a highly motivated and experienced individual to join our team as a Physical Health Liaison Practitioner in our Adult Learning Disability Service. As a key member of our team, you will play a vital role in ensuring that individuals with learning disabilities receive high-quality physical healthcare support.

Key Responsibilities

  • Provide physical healthcare support and liaison to professionals within the Learning Disability service and partner agencies.
  • Manage a small complex caseload of patients who have a learning disability and complex physical healthcare needs.
  • Work closely with the ULHT safeguarding lead nurse to coordinate patient care and provide support to Acute liaison nurses and primary care colleagues.
  • Support the validation of GP Quality Outcome Framework Learning Disability Registers annually.
  • Deputise in the absence of the physical health liaison lead and provide management and clinical supervision to staff within the service.

Person Specification

  • 1st Level Registration (NMC) or degree equivalent Allied Health Professional registration.
  • Mentorship Module.
  • Evidence of specialised continued professional training (degree level) in related clinical practice.
  • Experience of working with adults with a learning disability in healthcare settings.
  • Experience of working with people with Complex Physical Health needs within community and/or acute healthcare settings.
