Job Description

We are seeking a highly motivated individual to join our Learning Disability Service as a Learning Disability Physical Health Liaison Practitioner. This role involves providing physical healthcare support and liaison to professionals within the Learning Disability service and partner agencies.

Main Responsibilities
  • Ensure a high standard of physical healthcare support is provided for service users with a Learning Disability.
  • Manage a small complex caseload of patients who have a learning disability, working closely with ULHT safeguarding lead nurse and primary care colleagues.
  • Support the validation of GP Quality Outcome Framework Learning Disability Registers annually.
  • Provide management and clinical supervision to staff within the service and support training in community and acute physical healthcare.
About the Role

This is a community and hospital-based role providing physical healthcare support and liaison to professionals both within the Learning Disability service and partner agencies. The successful candidate will ensure a high standard of physical healthcare support is provided for service users with a Learning Disability.

Person Specification

The ideal candidate will have a background in healthcare and experience of working with adults with a Learning Disability. They will also have experience of managing complex caseloads and providing management and clinical supervision to staff.
