About the Role

We are seeking an experienced Healthcare Liaison Specialist to join our team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our Adult Learning Disability Service, you will play a vital role in ensuring that individuals with learning disabilities receive high-quality physical healthcare support.

Main Responsibilities

* Work closely with healthcare professionals to identify individuals with learning disabilities who require physical healthcare support
* Develop and implement tailored care plans to meet the individual needs of service users
* Collaborate with community and acute healthcare services to ensure seamless care transitions
* Provide expert advice and guidance to colleagues on physical healthcare best practices for individuals with learning disabilities

Requirements

* 1st Level Registration (NMC) or degree equivalent Allied Health Professional registration
* Mentorship Module and evidence of specialised continued professional training in related clinical practice
* Experience working with adults with learning disabilities in healthcare settings and managing complex physical health needs
