Physical Health Liaison Specialist

About the Role
We are seeking a highly motivated and experienced individual to join our team as a Physical Health Liaison Practitioner. This is an exciting opportunity to work at the forefront of adult learning disability services, contributing to the development of new ways of working across an integrated care system.
Key Responsibilities
- Provide physical healthcare support and liaison to professionals within the Learning Disability service and partner agencies.
- Manage a small complex caseload of patients with learning disabilities and complex physical healthcare needs.
- Work closely with the ULHT safeguarding lead nurse, coordinating patient care and providing support to Acute liaison nurses and primary care colleagues.
- Support the validation of GP Quality Outcome Framework Learning Disability Registers annually.
- Deputise in the absence of the physical health liaison lead and provide management and clinical supervision to staff within the service.
