Job summary

We are looking for 2 Band 6 Diabetes Specialist Nurse to join our team at Imperial College Healthcare NHS Trust (ICHT) which is comprised of five acute hospitals. Working as part of a multidisciplinary team with consultants, Diabetes Specialist Practitioners, podiatrists, dieticians and other professions, the diabetes nursing team perform a crucial role in making a difference to patients with diabetes.

The diabetes service at ICHT is a comprehensive acute service offering outpatient clinics specialising in:

Ante-natal care Enhanced type 1 service for insulin pump therapy and continuous glucose monitoring Structured education for type 1 diabetes Transition clinic for young adults Specialist foot unit Joint clinics for complex type 2 diabetes and obesity, liver and HIV Nurse-led clinics for young adults, type 1 diabetes and complex type 2 diabetes Post-holders will also need to support patients with diabetes on inpatient wards, Elective and non-elective admissions based on an e-referral system Links to community diabetes teams

Main duties of the job

In conjunction with the multidisciplinary team and in line with local and national targets provide specialist evidence based nursing advise to support and facilitate high quality, individualised care to patients/clients and relatives/ carers without supervision. Support and deputise for senior diabetes specialist nurses. This will involve dealing with complex situations and having to take appropriate action in a variety of settings. Provide expert advice and education for Healthcare professionals involved in the care of people with diabetes and their carers. Be flexible in the approach to the clinical area and to provide cover at all sites, if required, acting as professional role model providing direct and indirect clinical care, teaching and research.
