Key Responsibilities:
- Provide evidence-based clinical advice and guidance to healthcare professionals, patients, and their carers.
- Work collaboratively within the multi-disciplinary team to provide specialist care, advice, and education for patients with diabetes.
- Carry out diabetes and related diagnostic/therapeutic interventions as an independent practitioner.
- Interpret and write reports, and commence further possible treatment based on findings.
- Develop new skills and provide an expert, high-quality, nurse-led service.
- Provide ongoing follow-up care and support, including home visits, telephone support, and nurse-led clinics.
Requirements:
- Significant experience in diabetes nursing.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Strong clinical knowledge and skills.
