Role Summary

We are seeking a highly motivated and experienced Diabetes Specialist Nurse to join our team. The successful candidate will have a strong passion for delivering high-quality patient care and improving outcomes in line with national guidelines.

About the Job
  • Work in partnership with the NHS in primary care to deliver a unique nursing programme focused on driving best practice, delivering excellent patient care, and improving patient outcomes.
  • Collaborate with patients and practices to assist healthcare professionals in delivering up-to-date care for their patients, utilizing intimate knowledge of national treatment guidelines.
  • Develop and implement effective plans to improve patient outcomes, working closely with multidisciplinary teams to ensure seamless care delivery.
  • Conduct comprehensive assessments and develop personalized care plans for patients with diabetes, ensuring timely interventions and effective management of complications.
Requirements
  • Curent (NMC) Registration.
  • Diabetes Diploma or Equivalent.
  • Full UK Driving Licence.
