Diabetes Care Coordinator

We are seeking an exceptional Type 2 Diabetes Advisor to join our team. The successful candidate will be responsible for delivering high-quality care to patients with Type 2 diabetes, working in primary care across the country.
Responsibilities:
- Optimise oral hypoglycaemic therapy and conduct point-of-care testing to improve patient outcomes.
- Identify sub-optimally controlled patients and refer them for specialist insulin treatment where necessary.
- Contribute to the development of local and national Diabetes guidelines (NG28).
Requirements:
- A minimum of 3 years experience managing diabetes within primary care.
- IT literacy, including primary care systems.
- Full driving licence and current NMC registration.
Benefits:
The annual salary for this role is estimated at £45,000, plus a range of benefits including an annual bonus incentive scheme, car allowance, company pension, and private medical insurance.
