Type 2 Diabetes Care Coordinator

We are seeking an experienced Diabetes Specialist to join our client's nationwide team. This role offers a unique opportunity to develop a new diabetes service in primary care settings.
As a successful candidate, you will be responsible for managing patients with Type 2 diabetes, conducting clinic sessions in various locations, and providing education and support to improve patient outcomes.
To be considered for this role, you must have a Diabetes diploma or equivalent, at least 3 years' experience post-registration in managing diabetes within primary care, and a strong background in educating patients with Type 2 diabetes.
The salary for this position is £45,000 per annum, with additional benefits including:
- Annual bonus incentive scheme (worth up to 10% of annual salary)
- Car allowance or company car, depending on contract, plus fuel card for business miles
- Company pension, including salary sacrifice
- Private medical insurance
- NMC/RCN fees
- Monthly broadband contribution
- Daily lunch allowance when in practice
- 25 days annual leave, plus bank holidays (increasing after 5 years service), pro-rata for part-time
- Laptop and mobile phone provided
