Join Our Team: Chemistree Solutions is a leading service provider in healthcare strategy and delivery. We are seeking a Primary Care Diabetes Expert to join our team as a Nurse Advisor. In this role, you will work closely with patients and healthcare professionals to develop and implement effective diabetes management plans. Your expertise will enable you to drive improvements in patient outcomes and contribute to our organisation's mission to deliver measurable outcomes in Diabetes, Respiratory, and Cardiovascular disease areas.

Responsibilities: Deliver a range of services aimed at improving diabetes care and patient outcomes. Work in partnership with the NHS in primary care to support healthcare professionals in delivering up-to-date care. Provide education, support, and advice to people with Type 2 Diabetes, and develop strong relationships with patients and healthcare teams. Requirements: Minimum 3 years experience managing diabetes within primary care. Knowledgeable and confident in providing education, support, and advice to people with Type 2 Diabetes. IT literacy, including primary care systems. Current NMC registration, Diabetes Diploma or Equivalent, and full UK Driving Licence required. Estimated Salary: £50,000 - £60,000 per annum, depending on experience.
